Wedding DJ Song Request Template: Free Worksheet

Plan must-plays, do-not-plays, and wedding music cues with a free downloadable worksheet. Then set expectations for guest requests during the reception.

Separate the Couple's Plan from Guest Requests

A wedding music plan has two jobs: protect the important moments and give the DJ room to read the reception. Ask the couple for a short, prioritized list before the event. Collect guest suggestions separately so a late request does not override a ceremony cue or a do-not-play decision.

Use this worksheet in an email, a shared document, or your existing planning form. Requestify handles live event requests; this worksheet is a separate planning resource, not an automatic import or a built-in client intake form.

1. Confirm the Event and Decision Makers

  • Couple's names and pronunciation:
  • Wedding date, venue, and DJ contact:
  • Who can approve music changes on the day?
  • When will the couple and DJ review the final music plan?
  • Are there clean-version, cultural, language, or venue requirements?

Agree on a review date that gives the DJ time to source and check the exact versions. Do not treat a streaming link as confirmation that the performance file is ready.

2. Write Down the Key Music Cues

For each moment, record the song title, artist, exact version, start point, and who gives the cue. Mark “not applicable” where needed rather than leaving a blank that looks unfinished.

  • Ceremony entrance and exit, if the DJ handles the ceremony
  • Wedding party and couple's reception entrances
  • First dance and family dances
  • Any cultural or family traditions
  • Final song and any planned send-off

Include timing notes such as “fade after the first chorus” only after discussing them with the DJ. Keep the agreed cue sheet available to the DJ and coordinator.

3. Prioritize Must-Plays and Nice-to-Plays

Start with five must-plays and three nice-to-plays. Those numbers are a conversation starter, not a limit imposed by Requestify. Add artist names and versions, then explain why a track matters and where it fits: cocktail hour, dinner, or dancing.

Copy these columns into your music plan.
PrioritySong + artist + versionMomentWhy it matters
Must-play / nice-to-play[Your choice][Cocktail / dinner / dancing][Your note]

Also name favorite genres, decades, and the energy you want. A short brief gives the DJ more usable direction than a long list with no priorities.

4. Make the Do-Not-Play List Specific

Record songs, artists, or styles the couple wants excluded, and any version-specific exceptions. Confirm whether restrictions apply throughout the event or only during dancing. Guest enthusiasm does not cancel the couple's instructions.

Review the list with the DJ and keep it with the cue sheet. Requestify includes an event blacklist, but the DJ should still review the written brief and make the final set decisions.

5. Agree on Guest Request Wording

Try this on your request sign or in a short announcement:

“Have a song in mind? Scan the QR code to send the DJ a suggestion. We'll do our best to fit requests into the evening. The couple's music plan comes first, and a request does not guarantee a play.”

Decide whether guest requests are welcome during dinner, dancing, or both. If optional tipping is enabled, make clear that a tip is appreciation, not a promise that a song will play.

6. Test the Reception Request Flow

Create the event in Requestify, open its QR code print view, and scan the code from a guest's phone. Submit a test song and confirm it reaches the DJ queue using the connection available at the venue. Review requests between transitions, check popularity, and mark tracks as played as the night progresses.

Keep a direct event link on the sign and a paper or verbal fallback for connectivity problems. Follow the QR code song request setup checklist for printing, placement, and troubleshooting.

Open the DJ demo and guest demo in separate tabs. Submit a song from the guest demo and see how it reaches the DJ queue. The demos use sample event data and do not require an account.
